What Is A Design FMEA Failure Cause?

Design weakness, that if present, can lead to the Failure Mode. An example of a design weakness is a component material thickness being specified too thin. A common mistake made when filling out this column is placing a mechanism of failure (i.e. Sub-Assembly “A” fails to function.) in the Failure Cause column as a failure cause. This mistake is commonly made by FMEA teams who have been taught to create Design FMEAs for the assembly and every sub-assembly and component that makes up the assembly regardless of the number design groups.

Rich has introduced FMEA's for a new product development project I have been working on. His training of the method and the usage of his software was very clear and easy to follow.
